Chris, I found your post interesting. It touched on the area of tradeoffs
(particularly between safety and productivity). In some defence contracts
we have encountered reference to 'trade analysis'. It is a requirement to
define the tradeoffs between quality, safety, environment, security and
productivity, related to particular projects. Many will recognise these
as the popular 'flavour of the month' topics which cause regular crises in
many large organisations.
