David Hurst was looking for the source of that quite a while back and I
spent a lot of time trying to track it down--unsuccessfully. I managed to
trace it back to some articles in the 1970s but did not manage to find the
source. I did confirm that it did not originate with Joseph Luft, author
of the Johari Window (I spoke with him directly). Last time David and I
exchanged e-mails, he said he had a good lead on the source but I haven't
heard anything from him since.
