This sounds similar to the diagram I distributed in August (message
LO22409), depicting a graphic of how double loop learning might look. They

My diagram shows how one might continue in "single loop mode" until they
begin to realize that "something else" is causing them to fall short of
their intended outcome. That is when they might choose to do some
double-loop learning.
