As a theoretical observer, it seems to me that many failures are due to an
inability to come to terms with intrinsic conflicts within organisaitons -
and treating programs as isolated quick-fixes. TQM, for instance, may need
changes in managment styles that could be quite alien to the current
management, or inhibited by the corporate framework. Many of the quality
people I come across are reluctant to deal with that level of issue. I
found the follwing paper very enlightening.
