The main driver we have for LO is "do not repeat mistakes"

I am sure your organisation can find evidence of mistakes in on area
repeated in another. There will be costs associated with these repeats
which can be used to justify action to avoid them.

Now the big challenge is really to demonstrate that what you are doing is
actually reducing the likelihood of mistake repetition. This can be done
by finding examples of shared information which led to a change of
practice in one area as a result of a failure in another. If you cannot
find such examples, then you need more time, or your LO initiative is not
working as it should.
